For all new students, immunization records and a completed Student Medical History form is due by July 1st for the Fall Semester and January 1st for the Spring Semester.

1.  Immunization Records:

The State of North Carolina mandates that all students must provide their immunization records to the school they are attending. If this requirement is not met or not in process of completion, dismissal from school 30 days after registration is mandatory under North Carolina law.

The COVID vaccine is NOT required to attend Belmont Abbey College.  

**Students have the option to claim medical exemption if warranted or religious exemption from all vaccinations if this is their religious belief.  This is a right and privilege spelled out in the First Amendment of the Bill of Rights of our US Constitution and Belmont Abbey College is proud to stand up for this right.

2.   Student Medical History

3.   Physical Examination Form

ATHLETES are the ONLY students REQUIRED to get a yearly physical and their physical form is required to be uploaded to Athletics through SportsWare as directed by the Athletic Department. We would appreciate a copy for your Wellness Center chart if possible. 

All new students who are NOT participating in athletics are NOT REQUIRED to have a physical, but if you do get a physical, we would appreciate a copy for your Wellness Center chart as well.

4.  Student Athletes

Both Health Services and the Athletic Department have separate medical health forms. Both sets need to be completed and sent to their respective departments.

5.   Insurance

Each student must have medical insurance, either through a private insurance company plan or the insurance package offered through Belmont Abbey College. Information regarding the school’s medical insurance with information on how to waive the insurance will be sent during the summer months from Student Financial Services.

All students are billed every year on their fall tuition bill for the school health insurance. Students starting in the spring semester will be billed at a reduced rate for the spring semester.

All international students are required to keep the school-offered medical insurance.

For students who are US citizens, if you already have your own insurance and you wish to waive the school insurance, through the directives of Student Financial Services, you will need to complete an online waiver each year and show evidence of coverage by a private policy that provides benefits in this area.  To waive the school insurance click here.

We encourage the students to carry their insurance card with them or have a picture of the front and back of their insurance card on their phone in the event they need medical care outside the Wellness Center.
